Position Description
Assist in finding/identifying current member information by utilizing various means to determine phone and/or address
Perform community outreach activities to assist community based Disease Management (DM) program staff in locating members; inform eligible members of Disease Management/Care Management services available; promote and build trust for those services
Assist in coordinating members' relationships with multiple service systems
Assist members in obtaining a Medical Home and provide instruction on appropriate use of the Medical Home
Assist members in overcoming transportation barriers to obtain medical care
Promote and schedule appointments with members for Community Based RNs or telephonic Disease Management nurses
Inform staff and members about available community resources; assist members in utilizing the resources
Present health education in a culturally appropriate format
Provide social support to improve members' adherence to medical treatment and lessen the risk that they will drop out of program
Maintain confidential and accurate records of each member's address and telephone information, documentation of all contacts (and attempted contacts) and the type of assistance provided
Maintain a current, accurate database of community resource contacts. Maintain all client information in a confidential manner in accordance with policy and HIPAA regulations.
Location / Work Environment
Client Site and other locations as needed.
Position Qualifications
Must reside in and share experience, language and/or culture of the community to be served
Current valid driver's license and reliable transportation with required level of automobile liability insurance coverage
Minimum of two years' experience in health, social or community services
Current PPD
Bi-lingual preferred, but not required (Vietnamese and English)
Willingness and ability to travel to assigned operational areas/facilities
Experience using Windows operating system preferred
Ability to read, write and communicate effectively is critical
Strong organizational skills
Education and Professional Certifications
High School Diploma or GED
